A. The Life Extension Member Rewards Program and its benefits are offered at the sole discretion of Life Extension Foundation Buyers Club.
B. To participate, you must be a dues-paying member in good standing of the Life Extension Foundation. Expired, temporary or free trial members are not eligible for this program. You must also be a resident of the United States, Puerto Rico, a U.S. territory or Canada.
C. Services offered through program partners incur additional charges or fees. Life Extension has negotiated discounts or group pricing from these partners. Each program partner service comes with its own terms and conditions. See section VI for details on services and discounts offered. Contact the specific program partner for its terms and conditions.
D. Your Member Rewards category is determined on the date you renew your membership, based on your previous twelve months’ purchases. New members are automatically categorized as Valued Level. During the time period between your year-to-year renewal dates, your Member Rewards category cannot change.
E. Some program partner benefits are limited to specific states or geographical regions. Not all program partner services or discounts may be available to you.
F. The Life Extension Foundation Buyers Club may at any time add, delete or modify the Member Rewards program, category eligibility and benefits, in whole or in part, without prior written notice to participating members. Life Extension will make note of any program changes or additions via the Member Rewards web page at www.LifeExtension.com/rewards. Or you may call Member Care at 1-800-544-4440 to request a program update.
G. The Life Extension Member Rewards Program has no predetermined termination date. Life Extension may terminate this program or portions thereof at any time, without notice to participating members.
H. This brochure and these Terms and Conditions supersede all previous Member Rewards Program brochures and Terms and Conditions.
I. Life Extension is not responsible for typographical errors and/or omissions in this brochure.
J. These Terms and Conditions are governed by and construed under the laws (excluding conflicts of law provisions) of Florida. |
